How to make an appointment

  • Monday, Tuesday, Thursday 8:00 am - 5:00 pm
  • Wednesday 1:00 pm - 5:00 pm
  • Friday 9:00 am - 12:00 noon
  • We close 11:30 - 12:30 for lunch
• Our Location - 1938 Alabama Hwy 157 on the campus of Cullman Regional Medical Center. We are in a freestanding office with easy access and convenient parking.
• What to Bring to Your Appointment -
  1. A list of all medications you are currently taking as well as the dose.
  2. Health insurance information so we can file your claim on your behalf.
     • For HEALTHSPRINGS and MEDICAID, you must have a referral from your primary care physician
     • Insurance co-pays and deductibles are due when service is rendered.
     • We accept Mastercard, Discover, Visa, check or cash for your co-pay.
• Emergencies - We attempt to offer same day appointments but you are encouraged to call before 2:00 pm in order to facilitate that service.

  For emergencies that cannot wait until the following day or through the weekend, go directly to CRMC and have them call our physician on call.

• Unable to Keep Your Appointment? Please call as soon as possible so we can schedule another one convenient for you and open the appointment to someone else who may need it.
